Notification based on the Act on the Protection of Personal Information

In compliance with the Act on the Protection of Personal Information (hereinafter referred to as the "Act"), we will provide notification of the following items- including how "the person himself can easily know" and "the person himself can know (including cases of prompt reply as requested by such person)."

１．Items regarding notification of the purpose of use: （１）When personal information is obtained by document, e-mail, etc. from a customer, the purpose of use shall be clearly stated (Article 18 of the Act); （２）Storage of "personal information" obtained from customers is as follows.

２．Regarding "stored personal data", items deemed as those which "the person himself can know".

Purpose of use of "stored personal data" by our cooperative is as follows.

Types of "personal information" obtained by a bus parking reservation
(Name of company or organization making a reservation, telephone number, fax number, postal code, address, name of person of contact, group name or course number, destination, bus company name)
Purpose of use, DM, Sales activities, Marketing
Types of "personal information" obtained by a request for a pamphlet.
(Name, postal code, address, telephone number, mail address)
Purpose of use. To send a pamphlet
Types of "personal information" obtained by a regular parking contract (Name, postal code, address, phone number, vehicle type, car number, bank information)
Notice on the Purpose of the Use Facilities, Notice regarding overdue use fees
Types of "personal information" obtained by a campaign (Name, address, telephone number, circuit name)
Purpose of use: Campaign status confirmation

３．"Provision of personal information to a third party" (Article 23, paragraph 2-3 of the Act): Personal information is managed appropriately and will not be provided to a third party without obtaining customer consent, except in the following cases.

４．Items regarding procedures in response to a "request for disclosure" (Article 29 of the Act)

We respond to requests for disclosure, change or discontinuation regarding use from a person himself or his representative.

（１）Items requested for disclosure (information that contributes to specifying "stored personal data")Items deemed as "stored personal data" which is subject to disclosure are as follows.

（２）Request for Disclosure
To request disclosure, fill out the following application form and attach the necessary documents, submit in person or by mail. (When sending by postal mail, write "Request document enclosed" on the envelope.)

Shipping fee is the responsibility of the customer (Correspondences sent by this cooperative will be borne by the association.)

（３）Documents (forms) to be submitted for "Request for Disclosure"
When requesting disclosure, please use the following Application Form (A), fill out all the required information and submit it with identification documents (B)

Ａ　Application forms

Application form for disclosure of "stored personal data"
Application form for change of "stored personal data"
Application for discontinuation of use of "stored personal data"

Ｂ　Identification Documents

A copy of a legal document with a face photo, such as a driver's license or passport

（４）Request for disclosure by a representative

In cases where the person requesting disclosure of "stored personal data" is a legal representative of a minor or adult ward or has been designated by the person himself, in addition to the documents in the preceding paragraph, please submit the following documents (either A or B) as well.

Ａ　In the case of a legal representative

A document confirming legal representation right (copy of family register, etc.)
A document confirming legal representative for a minor or adult ward (copy of a legal document with a face photo, such as a driver's license or passport)

Ｂ　In the case designation as representative

Proof of power of attorney
Certification with the registered seal of the person (issued within three months, copies not accepted)

（５）Handling charges for a "Request for Disclosure" and method of collection

A fee of 200 yen (tax included) will be incurred for an application for disclosure of "stored personal data" per "item subject to disclosure request". In case of application by postal mail, please send as a registered mail for monetary transaction

No charges are incurred for an application for change of 'stored personal data' and discontinuation of use of 'stored personal data' and application are free.

（６）Response procedures to a 'Request for Disclosure'
A written reply will be sent to the address specified in the application form

（７）"Purpose of use" of personal data acquired for the "request for disclosure"

Personal data acquired in connection with the "request for disclosure" shall be handled only within the scope required for the request for disclosure. Documents will be stored for 2 years after completion of response to the request for disclosure and then deleted.

Reason for non-disclosure of stored personal data

Disclosure will not be made in the following cases. When such decision of non-disclosure is made, notification and reasons for such decision. A predetermined charge in non-disclosure cases will be incurred.

Addresses listed in application forms and an identification document
In cases where identification cannot be confirmed (address described in the application form and address listed in documents for identity verification do not match the registered address held by the company)
In cases of application by a representative, certification of representation cannot be confirmed.
In cases where application is not complete, or lacks the required application documents.
In cases where the requested matter for disclosure does not fall under "stored personal data".
In cases where there is risk of harm to life, body, property, or other rights and interests of the person himself or a third party.
In cases where proper implementation of operations by the association may be seriously hindered.
In the case of violation of other laws and regulations.
